I recognize this style/make but, for the love of coffee, can’t place it. Any leads would be appreciated.

Has the look of Milt Sparks construction to my eye, but Mitch Rosen calls this style the ARG.
I thought Galco used to have that style, but I can't find it. I don't think it is a Rosen here is the ARG Express https://mitchrosen.com/products/amer...-guard-express
El Paso Saddlery has the "C-Force" https://epsaddlery.com/product/c-for...force-holster/
It's also not an Andrews MacDaniel II http://andrewsleather.com/macdaniel2.htm

Thank you both. It does look very “Sparks” but I’m not familiar with their catalog changes, if any, over the past few years.

It is indeed a Galco NSA. Personally, I always suspected the designation stood for Not Strictly Ayoob's.
Good holster, actually. I have one for a Beretta 92. Works fine.
